Required forms for Check-In

1. Official NTSSA Team Roster (One copy signed by registrar and coach)

2. Transfer Rule: A team is limited to a total of five (5) transferred players per soccer year who were previously rostered with a club other than the club of which the team is a member (July 1, to Freeze date of State Cup Roster). A transfer is defined as adding a currently or previously rostered player onto your roster.

Any team that exceeds (5) transferred players must include a copy of all transfer release forms into the State Cup document upload as part of your required online registration. 

3. Virtual ID Cards for players and coaches will be used this year 
Teams that qualify for the 2022 US Youth Soccer Southern Regional Championships are required to use laminated North Texas Soccer ID Cards at the Southern Regional Championships.

4. Medical Release Forms
Current North Texas Soccer Medical Release forms for every player, including club pass players are required. All players are required to have medical release forms that will be verified when the team picks up their packet before the first game. Please do not upload medical release forms into GotSport. 

All required paperwork is to be submitted at the same time. No partial registration will be accepted.
Rosters must be uploaded into North Texas Soccer GotSport event by April 15, 2022. Do not send paperwork to North Texas Soccer
